Nitrofurantoin, an Excellent Empiric Choice for Outpatient Cystitis

Abstract: We read with interest the report by Sanchez et al. describing antibiotic resistance among the 10 most common pathogens isolated from urine cultures in women in the outpatient setting (1). Resistance rates reflected in the over 1.6 million isolates reported may exceed the prevalence of resistance in cases of uncomplicated cystitis in which a urine culture is not collected (2). However, we felt that these data provide important insight into a common clinical question: when a woman presents with urinary tract inf… Show more

“…For example, among female outpatients in 2012, susceptibility to ceftriaxone among urinary isolates was similar between girls aged 0–17 and women over 65 (83.4% and 84.3%), whereas for ciprofloxacin the rates of susceptibility dropped from 95.4% to 75% between those groups [4, 27]. A limitation to these surveillance data is that in the outpatient setting, a significant proportion of women may receive treatment for cystitis without a urine culture.…”

“…; Centers for Disease Control and Prevention, Atlanta, Georgia, USA b W e read the letter from Cortes-Penfield and Trautner (1) and thank them for their interest in our recent paper (2). In our paper, we demonstrated increasing resistance of Escherichia coli to ciprofloxacin and trimethoprim-sulfamethoxazole (TMP-SMX) in urinary isolates obtained from adult female outpatients, with the isolates retaining high levels of susceptibility to nitrofurantoin.…”
