Nocebo & placebo – o abordare etică | [Nocebo & placebo – an ethical approach]

Abstract

Introduction: Placebo & Nocebo are two aspects of medical practice less known and evaluated, although the ethical implications are complex and should be a subject of systematic analysis and even systematic research both from the point of view of medical staff, regardless of his training, of people involved in medical ethics. The purpose of the study was to analyse the general ethical aspects of the placebo and nocebo phenomena, and then the practical implications of the use of the two approaches were addressed. Material and method: Articles published in databases, such as Google Scholar, PUBMED, ISI, were analyzed. Results: The placebo effect was considered a gold standard, to which research for the introduction of a new treatment was reported, but later ethical analysis showed that the use of placebo raised a number of problems related to non-treatment. With the nuance of the analyzes, the introduction of psychological, behavioral type analysis appeared the description and analysis of the nocebo phenomenon, sometimes correlated with adverse events, negative manifestations. Conclusions: From an ethical point of view there will always be an attempt to answer the question: What is the ratio of the benefit of describing all the possible complications of a treatment and their minimization in order to be able to avoid the appearance of the nocebo phenomenon. Rezumat: Introducere: Placebo & Nocebo sunt două aspecte ale practicii medicale mai puțin cunoscute și evaluate, deși implicațiile etice sunt complexe și ar trebui să fie un subiect de analiză și chiar de cercetare sistematică atât din punct de vedere al personalului medical, indiferent de pregătirea lui, de persoanele implicate în etica medicală. Scopul studiului a fost să analizeze aspectele generale etice ale fenomenelor placebo și nocebo, urmând ca ulterior să fie abordate implicațiile practice ale utilizării celor două modalități de abordare. Material și metodă: Au fost analizate articole publicate în baze de date, precum Google Scholar, PUBMED, Clarivate - Web of Science. Rezultate: Efectul placebo a fost considerat un standard de aur, la care se raportau cercetările pentru introducerea unui nou tratament, dar ulterior analiza etică a demonstrat că folosirea placebo ridică o serie de probleme legate de non-tratament. Odată cu nuanțarea analizelor, introducerea analizei de tip psihologic, comportamental a apărut descrierea și analiza fenomenului nocebo, corelat uneori cu evenimentele adverse, manifestări negative. Concluzii: Din punct de vedere etic va exista în permanență încercarea de a răspunde la întrebarea: Care este raportul dintre beneficiul de a descrie toate complicațiile posibile ale unui tratament și minimalizarea acestora pentru a putea evita apariția fenomenului nocebo.
