Nociceptive flexion reflex and pain rating responses during endogenous opiate blockade with naltrexone in healthy young adults

Abstract: The effect of opioid blockade on nociceptive flexion reflex (NFR) activity and subjective pain ratings was examined in 151 healthy young men and women. Using a within-subjects design, NFR threshold was assessed on two days after administration of either placebo or a 50 mg dose of naltrexone. Electrocutaneous pain threshold and tolerance levels were measured after NFR threshold assessment on each day. Results indicated that administration of naltrexone was consistently associated with hypoalgesic responding. Sp… Show more

“…Nociceptive withdrawal reflexes have been elicited by electrical stimulation in many human experimental pain studies (Hugon, 1973;Willer, 1977;Petersen-Felix et al, 1996;Andersen, 2007;France et al, 2007). This is a very efficient stimulus for evoking with- Modulation of RRF by an intramuscular injection of capsaicin into the flexor digitorum brevis muscles.…”

“…Its mechanism of action in fibromyalgia related pain is hypothesized to be a result of endorphin receptor blockade with a resultant up regulation of receptor numbers. 20 Alpha-2 adrenergic receptor agonists: Tizanidine an (2A)-AR agonist is a muscle relaxant that was initially developed to manage the spasticity in multiple sclerosis. 21 It is the author's experience that Tizanidine is a useful "add on" drug to consider in fibromyalgia patients with poorly controlled pain, especially if there is a prominent component of myofascial pain and chronic daily headaches.…”

“…25 In some experimental human studies, the administration of naltrexone reduces pain measured on numerical pain ratings as well as the nociceptive flexion reflex in response to noxious electrical stimulation of the skin. 26 LDN reduces also relevant biological readouts for neuropathic pain, restores the analgesic effects of morphine and suppresses spinal neuroinfl ammation. Low doses of opioid antagonists enhance the analgesic effects of opioid agonists such as morphine in various animal models of chronic pain.…”
