Non Heart-Beating Donors in England

Abstract: Chaib E. Non heart-beating-donors in England. Clinics. 2008;63(1):121-34. When transplantation started all organs were retrieved from patients immediately after cardio-respiratory arrest, i.e. from nonheart-beating donors. After the recognition that death resulted from irreversible damage to the brainstem, organ retrieval rapidly switched to patients certified dead after brainstem testing. These heart-beating-donors have become the principal source of organs for transplantation for the last 30 years. The nu… Show more

“…Our primary non function rate was 3.2%, similar to that found by other teams [3,4,27], including cohorts of 'controlled donors' [2]. The delayed graft function rate for NHBD transplants is higher than in heart beating donor (HBD) kidneys, and is more frequent in uncontrolled donors [21] than in controlled donors as illustrated by a greater incidence of acute tubular necrosis [29].…”

“…The discard rate for uncontrolled donors in England was estimated to range between 50 and 65% [27]. Our final transplantation rate was 25% (63 potential NHBD/31 renal grafts).…”

“…While the use of kidneys and livers from DCD donors is increasing [9,10] , the use of DCD pancreata is still low (UNOS). DCD is not a novel concept.…”

“…Since 1968 and until now, procurement was limited to heart beating brain dead donors. The results of kidneys transplanted from non heart beating donors have significantly improved and nowadays are comparable to those of kidney transplantations from brain dead donors, thanks to a greater selection of donors and recipients and to improved prevention of cold and warm ischemia times as well as to several major therapeutic innovations (Alonso et al, 2005;Lee et al, 2005;Gagandeep et al, 2006;Kokkinos et al, 2007;Chaib, 2008). Procurement of non heart beating donors is therefore being reconsidered under considerations of feasibility, results and ethical and legal consequences.…”
