Non-Invasive Prediction of IDH Mutation in Patients with Glioma WHO II/III/IV Based on F-18-FET PET-Guided In Vivo 1H-Magnetic Resonance Spectroscopy and Machine Learning

Abstract: Isocitrate dehydrogenase (IDH)-1 mutation is an important prognostic factor and a potential therapeutic target in glioma. Immunohistological and molecular diagnosis of IDH mutation status is invasive. To avoid tumor biopsy, dedicated spectroscopic techniques have been proposed to detect D-2-hydroxyglutarate (2-HG), the main metabolite of IDH, directly in vivo. However, these methods are technically challenging and not broadly available. Therefore, we explored the use of machine learning for the non-invasive, i… Show more

“…Of particular note, our methodological expression of the IDH1 mutation represents a more physiological setting and is closer to that observed in patient tumors. 34 Further compounding the issue is the fact that the main 2-HG resonances overlap with the NAA and GLU signals in the in vivo MRS spectra at the magnetic field used (4.7 T) and are thus difficult to observe. 35 However, it should be noted that GLU synthesis was shown to be impaired with a higher production of 2-HG in gliomas, which is in line with our observations of reduced GLU in ex vivo HR-MAS and reduced GSH for both in vivo MRS and ex vivo HR-MAS in IDH1+ tumors.…”

“…Hence, it needs to be evaluated, if further PET-based analyses with the extraction of radiomic features may add value to the conventional image analysis in order to noninvasively identify the TERTp-mutational status. Interestingly, the predictability of key mutations using standard and advanced PET quantification also seems to vary depending on the used radioligands (53)(54)(55)(56)(57).…”

“…In addition, positioning of the voxel into the most representative part of the tumor was guided exclusively by standard MRI for the validation cohort, while voxel positioning had been also supported by O-(2- 18 F-fluoroethyl)-L-tyrosine ( 18 F-FET) positron emission tomography (PET) in the original cohort. Further technical details are given in the supplement ( Table S1 ) and for the original cohort in Bumes et al, 2020 [ 25 ].…”

“…In our previous prospective study cohort (original cohort), we focused on a full spectrum analysis of in vivo 1 H-MRS at 3.0 T in combination with a support vector machine (SVM) in 34 glioma patients. With this advanced diagnostic approach, we were able to achieve a specificity of 75.0% and a sensitivity of 95.5% for non-invasive determination of IDH mutation status [ 25 ]. This method demonstrated good technical feasibility and was well-tolerated by the patients with an additional examination period of approximately 20 min, making its use feasible in a standard clinical setting.…”
