Non-operative management of osteoarthritis of the knee joint

Abstract: Osteoarthritis is a chronic disorder of synovial joints in which there is progressive softening and disintegration of articular cartilage accompanied by the growth of osteophytes. Treatment designed for osteoarthritis should aim at reducing pain, improve joint mobility, and limit functional impairment. It can be achieved by pharmacological and non-pharmacological means. Non-operative treatment of OA is useful for patients with KL grade 1-3, which are early stages of OA. However, in an advanced stage of OA (KL … Show more

“…Platelet-rich plasma is under consideration as an innovative and promising tool with an effectiveness pattern comparable to the intra-articular administration of hyaluronic acid (7). Stem cell therapy is an additional strategy whose aim is to support the process of self-healing of the knee joint cartilage damage resulting from OA symptoms (1). The intra-articular application of stem cells, facilitating their differentiation into chondrocytes, is a process that includes the separation of the cells by centrifugation and other purification steps, with the aim of increasing cartilage buildup.…”

“…Osteoarthritis (OA) is a chronic disorder of synovial joints, in which there is progressive softening and disintegration of articular cartilage accompanied by other alterations, including osteophyte growth, cyst formation and subchondral sclerosis (1). It is the most common form of arthritis, and is the primary cause of disability and impaired quality of life in the elderly (2).…”

“…It has a variety of functions within joints such as shock absorption, energy dissipation, lubrication [20] and despite primarily being a structural protein it has also been shown to exert an anti-inflammatory effect [21]. Knees with OA have been shown to be deficient in hyaluronic acid (HA) which is a vital component of the synovial fluid and involved in lubricating all synovial joints [22]. McAlindon et al also note that introduction of exogenous HA also leads to increased endogenous HA production and the effects continue far beyond the duration of the exogenous administration [23].…”

“…After considering the risks and benefits of a total knee replacement (TKR), the patient opted for non-operative management of his knee osteoarthritis. A standard non-operative management protocol was followed [1]. This included non-steroidal anti-inflammatory drugs, physiotherapy and intra-articular injections of local anaesthetic and steroid.…”
