Non pigmenting mucosal fixed drug eruption due to tadalafil: A report of two cases

Abstract: Various ‘sex-stimulant’ medicines with fancy names and attractive packaging are available over the counter. Most contain phosphodiesterase 5 inhibitors in various strengths, often with herbal additions. These drugs are used erratically by the lay public, driven by folklore that such usage leads to increase in the length, girth or firmness of the penis. Such indiscriminate use by an otherwise healthy population leads to undue side effects.

“…One case had only oral mucosal involvement. 24 In this case, the patient’s inadvertent drug challenge, the recurrence of lesion on the landmark site, and the classic presentation of symptoms provide a strong argument for the association between tadalafil and FDE. The first step in diagnosing a cutaneous adverse drug reaction is taking a detailed medication history, including nonprescription and herbal products.…”

“…Nonpigmenting FDE has been reported in association with pseudoephedrine,[ 19 ] co-trimoxazole,[ 20 ] tetrahydrozoline, diflunisal, thiopental, piroxicam,[ 21 ] iothalamate, arsephenamine, paracetamol, intra-articular triamcinolone acetonide, eperisone hydrochloride,[ 22 ] furazolidone, and acetaminophen. Recently, reports of nonpigmenting FDE to eprazinone, sorafenib,[ 23 ] tadalafil,[ 24 ] esomeprazole, and fluoroquinolones[ 25 ] have also been described. The lesions can be as large as over 10 cm and multiple lesions possibly represent abortive variant of toxic epidermal necrolysis (TEN).…”
