SummaryBone deficits of the jaws are often attributed to accidents, surgical removal of benign lesions or malignant neoplasms, congenital abnormalities, periodontal inflammation, tooth abscess or extraction and finally jaw atrophy due to advanced age or general disease.These bone defects require rehabilitation for a variety of reasons, e.g. maintaining the normal anatomic outline, eliminating empty space, aesthetic restoration and placing dental implants. Today, several techniques have been developed to eliminate these bone deformities including bone grafting, guided bone regeneration, distraction osteogenesis, use of growth factors and stem cells.Bone grafts consist of materials of natural or synthetic origin, implanted into the bone defect site, documented to possess bone healing properties. Currently, a variety of bone restorative materials with different characteristics are available, possesing different properties.Despite years of effort the ‘perfect’ bone reconstruction material has not yet been developed, a further effort is required to make this objective feasible.The aim of this article is to provide a contemporary and comprehensive overview of the grafting materials that can be applied in dentoalveolar reconstruction, discussing their properties, advantages and disadvantages, enlightening the present and the future perspectives in the field of bone regeneration.

Os objetivos e os critérios para uma reconstrução mandibular bem sucedida permitem o estabelecimento de continuidade óssea com forma e largura adequadas do arco, altura alveolar, e melhores contornos faciais, restaurando as capacidades funcionais e sociais dos pacientes. Embora muitos cirurgiões contemporâneos defendam o uso de retalhos livres vascularizados para a reconstrução de defeitos maxilofaciais, é fundamental enfatizar o papel que os enxertos ósseos não vascularizados exercem, pois trata-se de uma opção excelente para melhorar a aparência facial e configura-se em uma abordagem viável para o fornecimento de altura óssea satisfatória. Este presente trabalho tem como objetivo evidenciar as perspectivas atuais encontradas na literatura acerca das reconstruções de defeitos mandibulares centrais e laterais com enxertos ósseos autógenos não vascularizados. Trata-se de um estudo de revisão e exploração das bases de dados PubMed/Medline, Embase, Scielo e Google Schoolar, sem restrição de tempo, mas com compilação dos estudos publicados nos últimos seis anos. Os achados da literatura evidenciaram que embora não haja um consenso na literatura, existem estudos publicados sobre o tema trazendo defeitos mandibulares acima de 9 cm tratados com sucesso e que utilizaram de técnicas inovadoras a fim de propiciar o melhor planejamento e manejo cirúrgico ao paciente. Background: Bone substitutes, either from human (autografts and allografts) or animal (xenografts) sources, suffer from inherent drawbacks including limited availability or potential infectivity to name a few. In the last decade, synthetic biomaterials have emerged as a valid alternative for biomedical applications in the field of orthopedic and maxillofacial surgery. In particular, phosphate-based bone substitution materials have exhibited a high biocompatibility due to their chemical similitude with natural hydroxyapatite. Besides the nature of the biomaterial, its porous and interconnected architecture is essential for a correct osseointegration. This performance could be predicted with an extensive characterization of the biomaterial in vitro. Methods: In this study, we compared the biological, chemical, and structural features of four different commercially available bone substitutes derived from an animal or a synthetic source. To this end, µ-CT and SEM were used to describe the biomaterials structure. Both FTIR and EDS analyses were carried out to provide a chemical characterization. The results obtained by these techniques were correlated with cell adhesion and proliferation of the osteosarcoma MG-63 human cell line cultured in vitro. Results: The findings reported in this paper indicate a significant influence of both the nature and the structure of the biomaterials in cell adhesion and proliferation, which ultimately could affect the clinical performance of the biomaterials. Conclusions: The four commercially available bone substitutes investigated in this work significantly differed in terms of structural features, which ultimately influenced in vitro cell proliferation and may so affect the clinical performance of the biomaterials.
