Nondisclosure of Complementary and Alternative Medicine Use to Primary Care Physicians

Abstract: Although one-third of US adults report using complementary and alternative medicine (CAM), integration of CAM into the conventional medical system is inconsistent. 1 Patients have shown a desire for their primary care physicians to inquire about CAM and refer to CAM practitioners (acupuncturist, massage therapists, etc), but primary care physicians rarely initiate conversations with patients about their use of CAM. 2,3 Patients have also expressed concerns about discussing the use of CAM with their physicians,… Show more

“…This is particularly salient when discussing DI-AIH, given that 23% of patients with AIH are >60 years of age 20. Because a lack of physician inquiry or knowledge about CAM use is a reason frequently cited by US adults for non-disclosed use of NVNM DS or other CAM modalities,7 greater vigilance on the part of healthcare providers is needed, particularly when caring for at-risk populations with a high prevalence of NVNM use.…”

“…Therefore, in this study it is unknown whether disclosure was patient initiated or provider initiated, or how positive CHA experiences and beliefs about CHA may be related to each type of disclosure. Indeed, there is evidence that CHA use is not likely to be disclosed if the provider doesn’t initiate the discussion (22). Accordingly, future work on CHA disclosure behaviour should take a more fine-grained approach to assessing the circumstances surrounding CHA disclosure, and employ a longitudinal approach to test the temporal precedence of CHA values in relation to disclosure suggested by the CAM consumer commitment model (30).…”

“…Understanding the factors that can facilitate disclosure of CHA use can help to guide educational strategies for both CHA users and conventional health-care providers, as well as inform clinical guideline practices. Yet to date, the research on CHA disclosure has been largely atheoretical and focused on barriers to disclosure, such as poor patient-provider communication (17, 2022), rather than on factors that might promote or facilitate disclosure of CHA use, such as beliefs and outcomes from CHA use. The aim of this study was to address this gap by applying recent theoretical developments on CHA-related behaviours to understand the implicit reasons for disclosure of CHA use using a large, nationally representative sample.…”

“…The rate of nondisclosure obtained in this study is comparable to the results of previous reports in the literature which showed that a significant proportion of PLWHA are reluctant to discuss their use of CAM with the physician (53% in Ontario, Canada [15]; 68% in Malaysia [27]). Potential reasons for not discussing the CAM use with the physician could be related to physicians not asking about CAM use or to concerns about physician knowledge regarding CAM rather than to physician discouragement or negativity about the use of CAM [64]. Additional reasons may include patients' concerns for disproval and loss of medical care privileges, in addition to the belief that CAM is safe, holistic, natural, and nontoxic in contrast to conventional medicine, which is usually perceived as depersonalized and not completely effective [65].…”
