Nonsurgical Management of the Neurogenic Bladder in Children

Abstract: The neurogenic bladder can have many serious consequences. However, with proper understanding of the disease spectrum, and with proper awareness of the surveillance and management tools, adverse outcomes can be minimized. The incidence of renal failure is uncommon. With proper medical therapy, one can achieve continence and minimize the risk of symptomatic urinary tract infections. There are a number of nonsurgical strategies described that can help in the management of the neurogenic bladder.

“…These patients are often treated with clean intermittent catheterization (CIC) to decrease the risk of unsafe bladder pressures and ensure adequate bladder emptying. Patients who perform CIC for neurogenic bladder frequently develop chronic bacterial colonization, also known as asymptomatic bacteriuria [ 1 , 2 ]. Chronic asymptomatic colonization does not require antibiotic treatment.…”

“…Concordando com Molin et al, (2018), Kryger (2008) estudou o gerenciamento não cirúrgico da bexiga neurogênica em crianças, os resultados mostraram que o manejo da (BN) evoluiu nos últimos 30 anos bastante. Para o autor com gerenciamento adequado, a incidência de insuficiência renal é rara.…”

“…The most frequent causes of reduced functional or anatomical BC in children are neurogenic dysfunction (NVD), posterior urethral valves (PUVs), and bladder exstrophy (BE). When more conservative methods ( 1 ) such as medications or injection of Botulinum toxin fail, surgical bladder augmentation is needed to achieve continence and prevent or correct hydronephrosis.…”
