Nontraumatic Spinal Epidural Hematoma Associated with Clopidogrel

Abstract: A 73-year-old female treated with clopidogrel for vascular disease presented with sudden onset of back pain, urinary retention and paraplegia. MRI scans demonstrated a thoracolumbar epidural hematoma and the patient underwent emergency laminectomy for evacuation of the hematoma. A possible causal link between clopidogrel and occurrence of the hemorrhage is discussed and the literature on spontaneous spinal hematomas is reviewed.

“…36 The potency of these medications is demonstrated by recent reports of spontaneous spinal hematomas during clopidogrel therapy. [226][227][228] Ticagrelor Ticagrelor (Brilinta) represents a new class of nonthienopyridine platelet inhibitors designed to address the limitations of current oral platelet drugs. Ticagrelor completely but reversibly inhibits ADP-induced platelet activation, unlike the thienopyridines (eg, clopidogrel, prasugrel).…”

“…[18][19][20][21] Several case reviews report that this risk is much more than just theoretical for patients on aspirin, clopidogrel, or a combination of the two. [22][23][24][25][26][27] For these reasons, spinal surgeons have a vested interest in seeking out any variables that they may be able to adjust to mitigate these risks while still doing what is ultimately best for the overall health of the patient.…”
