Normal Physiologic and Benign Foci with F-18 FDG Avidity on PET/CT in Patients with Breast Cancer

Abstract: Altered physiologic and benign F-18 FDG uptake in the lower cervical muscle and chest wall following ipsilateral breast surgery or radiotherapy were common, and also normal physiologic uptake in ovary and uterus, brown fat, thyroid were considered as predominant findings in women patients with breast cancer. Knowledge of these findings might aid in the interpretation of FDG PET/CT in patients with breast cancer.

“…Although 18 F-FDG PET/CT shows a low false-positive rate for the staging of breast cancer, careful attention must be paid to normal or altered physiological 18 F-FDG uptake patterns and benign hypermetabolic disease, in order to avoid misinterpretations. As observed in the present study, the most common sites of increased 18 F-FDG uptake in female patients with breast cancer are the thyroid glands, where hypermetabolic disease occurs, and the ovary/uterus, where there is normal physiological uptake ( 21 ) . However, abnormal 18 F-FDG uptake should be evaluated in order to exclude metastases and non-breast second primary tumors, which are occasionally encountered in breast cancer patients ( 22 , 23 ) .…”

“…In a number of cases, the intensity of FDG accumulation is used to differentiate between malignant and benign lesions (23)(24)(25)(26). However, numerous other benign conditions, including abscess, pulmonary granuloma, tuberculosis and sarcoidosis, may also present with increased 18 F-FDG uptake (23,(27)(28)(29)(30)(31)(32). Thus, there is a high chance of encountering false-positive conditions in a clinical setting.…”

“…When a 2-[ 18 F]FDG-avid breast lesion is incidentally detected in a study performed for reasons other than BC, further characterization with diagnostic mammography and/ or breast US should be performed because these lesions are malignant in 30-40% of cases [62,63]. Several aetiologies have been described, including unsuspected BC, lymphoma, and metastases [59,[64][65][66][67].…”
