Novel Approach to a Giant External Iliac Vein Aneurysm Secondary to Posttraumatic Femoral Arteriovenous Fistula

Abstract: We describe a case of a 55-year-old male with a remote history of a gunshot wound to the left thigh who presented with a 1-year history of worsening high-output congestive heart failure, left lower extremity edema, and left lower abdominal discomfort. Diagnostic evaluation included a computed tomographic angiography (CTA) that demonstrated a fistulous communication between the left superficial femoral artery (SFA) and vein (SFV) as well as a 7.2-cm external iliac vein aneurysm. Given his symptomatology, an end… Show more

“… 1 An iliac arteriovenous fistula (AVF) is also a rare condition, mostly caused by traumatic, congenital, or iatrogenic origin. 2 There have been some reports about favorable outcomes of MTS treated by endovascular therapy; however, MTS coexisting with iliac AVFs is rare, and appropriate treatment has not been established.…”

“…Traumatic arteriovenous fistulae can be repaired by open or endovascular approaches [ 5 , 6 ]. This patient's fistula was best managed with an endovascular approach but there were limitations given the proximal and distal landing zones were significantly different in size.…”

“…Endovascular and hybrid managements of iliac vein aneurysms are reported in six cases in the literature review 3,26,39,43,44,48 (Table 6). AVF exclusion by arterial stent graft is the proposed method for iliac aneurysms resulting from traumatic AVFs.…”

“…Then, the size of aneurysm should be followed-up. 3,43 If open closure of AVF is considered, the venous aneurysm is then could be excluded by a venous graft. 39 Traumatic iliac vein pseudoaneurysms are managed by trans-catheter embolization.…”
