Novel Clinical Trial Designs to Improve the Efficiency of Research

Abstract: SUMMARY Large randomized trials provide the highest level of clinical evidence. However, enrolling large numbers of randomized patients across numerous study sites is expensive and often takes years. There will never be enough conventional clinical trials to address the important questions in medicine. Efficient alternatives to conventional randomized trials that preserve protections against bias and confounding are thus of considerable interest. A common feature of novel trial designs is tha… Show more

“…The outcome in terms of 1‐ and 5‐year survival is under way. Presumably, new RCTs may be sparse in numbers, since such studies are long lasting, cumbersome and expensive to carry out 27 . A large observational study may then be a complement to the results of an RCT.…”

“…Novel trial designs facilitate enrollment of large numbers of patients at modest cost, offering greater opportunities to demonstrate clinical effectiveness. 26 The conventional parallel, 1:1 randomized controlled trial should be familiar to all readers, but other common designs include crossover, cluster, and factorial trials. These are not new but remain uncommonly used in anesthesia research.…”

“…Efficient alternative study designs to traditional in-person assessments could emphasize convenience for families with young children including, where appropriate, validated telephone or telehealth interviews for parent-and patient-reported outcomes. Although desirable to families and fiscally efficient, flexibility by regulatory bodies is needed to accept such alternative nontraditional study design methods, 15 especially as many of these methods are undergoing validation research during the COVID-19 pandemic.…”
