Novel insights into fecal incontinence in men

Abstract: Fecal incontinence in men is common, yet data on sex differences in clinical features, physiology, and treatment are scarce. We provide evidence that men, compared with women, with fecal incontinence have unique clinical features and physiology and are less likely to have investigations and treatment despite successful outcome with anorectal biofeedback therapy.

“…In this work, the etiological profile of refractory FI in men was different from that in women, including predominantly anal surgery and LARS. These data are, in part, comparable with those reported in the literature . The factors leading to FI in men are generally less well‐studied than those in women.…”

“…One might ask if this proportion is well‐representative of the proportion of men with FI. In the unit of Rennes, 11.1% of referred FI patients were men and were of similar age to women, which is in‐line with previous publications . It is thus interesting to consider why fewer men seek consultation for FI.…”

“…The causes of RI are multifactorial, but a leading cause in humans is an aging-associated decrease in the IAS tone (AADI) and compromise in the fibroelastic properties (FEP) of the IAS 57 . Little is known about the molecular mechanisms underlying these phenomena, and this has impeded the development of a specific and safe treatment for debilitating AADI-related RI 5,8 .…”

“…However, because anorectal pressures during evacuation overlap between DD and healthy subjects, further studies are necessary to understand the relative contribution of manometric findings to the pathophysiology of DD 6 . ARM is also useful for identifying anal weakness and rectal sensory disturbances in fecal incontinence (FI), 7,8 and for assessing reflex relaxation of the IAS in response to rectal distension in patients being tested for Hirschsprung disease, as well as those with megarectum or megacolon 9,10 …”
