Pelvic fractures (PFs), involving the disruption of the bony structures in the pelvic region, are complex injuries often associated with high-energy trauma. Such fractures can significantly impact a patient's mobility and overall quality of life. Concurrently, fractures of the malleoli, specifically the lateral and medial aspects of the ankle, are common lower extremity injuries that can result from various mechanisms, including twisting or direct trauma. This case report presents the multidisciplinary approach employed in the successful treatment of a 26-year-old male patient with a rare combination of PFs involving both anterior and posterior columns, along with lateral and medial malleolus fractures. The patient underwent a series of surgical interventions to stabilise the fractures, followed by a tailored physiotherapy management plan. The report discusses the postoperative care strategies and the crucial role of physiotherapy in the rehabilitation process.

Revision surgery in comminuted distal one-third femur fracture using retrograde intramedullary nail: A case report

Fractures of the distal one-third of the Femur bone are quite common nowadays due to the increase in the incidence of road traffic accidents, falls, sports and outdoor activity, etc. Retrograde intramedullary nailing has been considered a good surgical intervention for the management of such cases.Cement augmentation and shape memory alloy is also preferred to provide mechanical stability at the site of fracture. However, in the case of comminuted fracture, this modality of surgery cannot be used as the first option of choice. The present case report of 18 years old male who sustained a comminuted fracture of the distal one-third of the femur reviews the progression of the case after the first surgery using open reduction and internal fixation and plating and postoperative complication of nonunion after dynamization. It also describes the outcome of revision surgery using the retrograde intramedullary nail.

Accidents on the highway as well as falls have become more common with the expansion of the transportation and construction industries. Patients have traditionally been instructed to use protective weight-bearing during the early rehabilitation phase following a well-fixed distal femoral fracture. Clinical finding: The patient had complained of pain and swelling over the thigh's lateral surface and unable to flex the knee joint and inability to bear weight on left lower limb pain on NPRS 8/10 on movement and on rest 6/10. Diagnosis:The X-ray reveals that comminuted supracondylar femur fracture and the CT report reveal that comminuted displaced fracture in the distal end of the femur, extending to involve lateral, medial condyles, inter condylar region and the articular margin. Conclusion: In this case, the rehabilitation routine helped the patient regain functional independence at home and at work by reducing pain and tenderness while also improving range of motion, muscle strength and endurance.
