Novel ultrasound-based volume estimation of prostatic benign enlargement to improve decision-making on surgical approach

Abstract: Aim: To assess the precision of preoperative ultrasonography (US)-determined prostate volume and to propose formulas for improving it. Methods: This retrospective study comprised 155 consecutive men who underwent open prostatectomy for benign prostatic hyperplasia (BPH) between 2013 and 2019. Preoperative prostate volume was estimated by either abdominal US (AUS) ( n = 92) or transrectal US (TRUS) ( n = 63), and was compared with the weight of surgically enucleated tissue at a conversion rate of 1 ml (US) = 1 … Show more

“…As for tools for assessment, previous literature demonstrated that although both transabdominal and transrectal ultrasounds could provide information to predict the resected weight of prostate, the latter could provide the closer approximation. 12 In our multivariable logit model, assessment of prostatic volume via transrectal ultrasound could also be associated with iPCa.…”

“…Systematic biopsies were done via transrectal ultrasound-guided fashion and prostatic volume was measured by transrectal ultrasonography (height ×width ×length × 0.52) before surgeries. 12 Information regarding family histories of prostate cancer was collected from grandfather to their brothers.…”

“…Four studies claimed otherwise [24], [29], [40], [41]. One study stated that SUS-derived PV estimates signi cantly over-estimated specimen weight [29], while another claimed SUS-derived PV estimates were signi cantly different to specimen values [24].…”

“…suggests that the constant value of the geometric model may not be the most accurate formulation, and that instead a "variable constant"" (possibly a function of the prostate size) should be used. Such a formula has been presented, where the "adjusted prostate volume" is a function of patient age and the calculated prolate ellipsoid volume of the prostate [24].…”

“…Based upon pathologic specimens during an open prostatectomy, an AUS prostate volume-adjusting formula was calculated to improve the accuracy of AUS volume determinations [6]. Considering the global trend toward shifting from open enucleation to transurethral enucleation, this is a validation study to investigate the accuracy of that formula in predicting prostate adenoma volumes and in improving preoperative planning before transurethral laser enucleations of BPH.…”
