Background Prevention of falls during hospitalization depends in part on the behaviors of alert patients to prevent falls. Research on acutely ill patients’ intentions to behave in ways that help prevent falls and on the patients’ perceptions related to falls is limited. Objective To explore hospitalized adults’ perceptions related to risk for falling, fear of falling, expectations of outcomes of falling, and intention to engage in behaviors to prevent falls. Methods Adult, alert, acutely ill inpatients (N = 158) at risk for falling completed a survey consisting of 4 scales and 3 single items. Nurses’ assessments and patients’ perceptions of the risk for falling were compared. Results Decreased intentions to engage in behaviors to prevent falls were correlated with patients’ increased confidence in their ability to perform high-risk behaviors without help and without falling (P < .001), decreased fear of falling (P < .001), and decreased perceived likelihood of adverse outcomes if they did fall (P < .001). Although nurses’ assessments indicated a risk for falls, 55.1% of the patients did not perceive a high likelihood of falling while hospitalized. Whereas 75% of patients intended to ask for help before getting out of bed, 48% were confident that they could get out of bed without help and without falling. Conclusions Although assessments may indicate a risk for falling, acutely ill inpatients may not perceive they are likely to fall. Patients’ intentions to engage in behaviors to prevent falls vary with the patients’ fall-related perceptions of confidence, outcomes, and fear related to falling.

Objectives:The purpose of this study was to understand the relation among fall knowledge, fall attitude, nursing environment and fall prevention activities in nurses who work at geriatric hospitals, and identify factors which affect on fall prevention activities. Methods: Using the convenient sampling method, the data were collected from nurses who had been working at 16 geriatric hospitals in D Metropolitan City for longer than three months. The collected data from 210 nurses were analyzed using descriptive statistics, independent t-test, one-way ANOVA with Scheffė test and multiple regression with SPSS/WIN 20.0. Results: Subjects' fall knowledge and fall attitude turned out to work as relatively high influence factors. They perceived nursing practice environment as positive and highly involved in fall prevention activities as well. Geriatric hospital nurses who were older, married, paid better, more experienced in clinical environment and learned more about fall-related education were better at fall prevention activities. Fall prevention activities were significantly related to fall attitude and nursing practice environment. Under the control of individual characteristics among geriatric hospital nurses, Hierarchial Linear Regression revealed that fall attitude, wage and fall education hours worked as positive influence factors, however, nursing practice environment did not affect much. Explanation power of the variables were 30.4%. Conclusions: Based on the result of the study, an education program with sufficient hours to change nurses' fall attitude needs to be developed, and a plan which will bring positive change in nursing practice environment needs to be sought to improve geriatric hospital nurses' fall prevention activities.

Challenges to develop and evaluate any interventions to eliminate risk factors for falls often relate to selecting feasible interventions and whether staff members accept the interventions and adhere to adopting the intervention. Using multiple data sources with time factors to cross-validate the sufficiency of nurses' knowledge with their practice patterns may be more productive. This need further supports the importance of this paper about possible data sources used in the research on patient safety specific to fall reduction for adults in hospital inpatient care settings.
