Nutritional Management of Patients Undergoing Surgery following Diagnosis with Encapsulating Peritoneal Sclerosis

Abstract: Background Encapsulating peritoneal sclerosis (EPS) is a rare but serious complication of peritoneal dialysis (PD). Gastrointestinal (GI) symptoms affect appetite and dietary intake. Adequate nutrition is especially important if surgical interventions are required. Aim To investigate the nutritional management of 23 EPS patients that underwent surgical intervention between 1999 and 2005 at Manchester Royal Infirmary, United Kingdom. Methods EPS was recognized by GI symptoms and diagnostically confirmed by lapa… Show more

“…As most patients with EPS suffer from malnutrition, appropriate nutritional support is imperative in the management of these patients. Studies from our centre have demonstrated poor pre-operative nutritional status and improved survival with peri-operative Total Parenteral Nutrition (TPN) in patients with advanced EPS who underwent surgery (peritonectomy and enterolysis) (De Freitas et al, 2007 ; Campbell et al, 2014 ). However, TPN therapy on its own does not have any curative effect as demonstrated by a study from Japan which showed no recovery in patients treated with TPN alone (Kawanishi and Moriishi, 2005 ).…”

“…Severe complications, including intra-abdominal infection, intestinal fistula, short bowel syndrome, and bowel perforation in idiopathic cocoon abdomen are not observed. Early postsurgical activity is recommended, and the postoperative dietary guidance is also very important to their rehabilitation [21]. The diet should be gradually restored according to their gastrointestinal function.…”
