Observations on the systemic effect of topical clobetasol propionate (Dermovate).

Abstract: SummaryTopical application of clobetasol propionate in a strength of 0 05% in cream or ointment (Dermovate) suppressed the hypothalamic-pituitary-adrenal axis in both normal people and patients with diseased skin. In normal people the 9 am serum cortisol level was suppressed when between 45 and 90 g was applied weekly. Insulin stress test results in patients with diseased skin showed an abnormally small response after long-term application of similar amounts. We conclude that long-term administration should be… Show more

“…Furthermore, percutaneous absorption of potent and very potent corticosteroids has been reported to lead to suppression of the pituitaryadrenal axis. 16 Because repigmentation is a long-term process, and because of these well-known serious adverse effects of corticosteroid treatment, controlled long-term studies on repigmentation with potent corticosteroids have not been conducted.…”

“…Feiwel et al [6] have reported the occurrence of diminished adrenal response in children treated with topical steroids under stockinette occlusion and Carruthers et al [4] have noted impaired responses in insulin stress in adults treated without occlusion.…”

“…Although topical corticosteroids are prescribed worldwide many times per day, corticosteroid-induced adrenal suppression has rarely been seen and reported on clinical investigation [16,17]. Appropriately used topical corticosteroids may lead to intermittent suppression of the HPA axis, with no clinical implications.…”

“…Appropriately used topical corticosteroids may lead to intermittent suppression of the HPA axis, with no clinical implications. Carruthers et al [16] showed that suppression of the HPA axis due to topical treatment with clobetasol propionate 0.5% occurs in a dose-dependent manner. However, under similar conditions the majority of persons with a healthy skin (given 90 g/week) showed adrenal suppression.…”
