Observer’s Assessment of Alertness/Sedation-based titration reduces propofol consumption and incidence of hypotension during general anesthesia induction: A randomized controlled trial

Abstract: Administration of a single propofol bolus dose for anesthesia induction causes hypotension. We included 160 patients (74 males and 86 females; mean age, 42.4 ± 10.7 [range: 18–60] years) with the American Society of Anesthesiologists status I–II undergoing elective surgery under general anesthesia. Using simple randomization, the patients were divided into a conventional group ( n = 80; received 2 mg/kg propofol at a rate of 250 mg/min) and titrated group ( n = 80; received propofol at a rate of 1 mg/kg/min un… Show more

“…And two RCTs [ 12 , 15 ] suggested that remimazolam was safe and effective compared with propofol for induction in cardiac surgery with less hemodynamic fluctuations. However, propofol has a huge potential to cause hypotension, and its incidence could reach more than 70% during general anesthesia induction [ 26 , 27 ]. It indicates that propofol may be inappropriate for patients with impaired cardiac function, consequently making the benefit of remimazolam on cardiovascular function less convincing when comparing with propofol.…”

“…Arterial hypotension occurring during general anesthesia significantly increases mortality and contributes to organ dysfunction during the perioperative period. 1 , 2 , 3 , 4 The duration and severity of hypotension are closely linked to the type and extent of organ dysfunction. 1 , 2 , 3 , 4 , 5 Maheshwari et al 6 revealed that nearly one-third of intraoperative hypotension episodes occur during the time between anesthesia induction and the start of surgery.…”
