Obstructive azoospermia secondary to bilateral epididymal cystadenomas in a patient with von Hippel-Lindau

Abstract: von Hippel-Lindau (VHL) is a heritable cancer syndrome associated with findings in multiple organ systems. Male patients can be affected by epididymal cystadenomas which are benign tumors localized to the epididymis. While benign, these tumors can cause pain and in very rare circumstances can have an effect on fertility especially when present bilaterally. We present a case of a young man with obstructive azoospermia secondary to bilateral cystadenomas, with a focus on his work-up and management.

All scenarios should be discussed, and patients should be supported in their decision with consideration of their cultural/religious backgrounds.…”

“…Regarding the infertility problem in this specific setting, standard approaches to obstructive azoospermia, which include sperm retrieval via percutaneous or open biopsy of the testes, are the preferred management strategies (15). Symptomatic PCE, instead, when feasible, is usually treated with testicle-sparing surgical…”

“…Regarding renal lesions in VHL, kidney may develop thousands of microscopic or large lesions that are solid or cystic with each lesion having malignant potential. The average age for an initial lesion is the 4th decade of life, but many showing lesions much earlier [ 21 , 22 , 23 , 23 ]. However, one study described a cohort where the median age of onset for an initial lesion was age 28, with a median life expectancy of 66.…”
