Occlusive Cerebrovascular Disease with Moyamoya Vessels and Intracranial Hemorrhage during Pregnancy

Abstract: A 24-year-old female in the sixth month of pregnancy developed headache and vomiting caused by an intraventricular hemorrhage from moyamoya vessels. Superficial temporal artery-middle cerebral artery bypass surgery was performed when she was in the seventh month of pregnancy in order to pre vent intracranial rebleeding or a cerebrovascular ischemic attack during labor and delivery. The pa tient delivered successfully at full term, without complications. The clinical features and management of patients with suc… Show more

“…We excluded moyamoya syndrome secondary to irradiation of the head, Down syndrome, brain tumor, neurofibromatosis and meningitis. Intracranial hemorrhage from moyamoya disease has been infrequently reported during pregnancy, and is exceptional in a Caucasian pregnant woman in comparison with Asian women [4,5,6,7]. There is no firm evidence that pregnancy in patients with moyamoya disease is a risk factor for intracranial hemorrhage or cerebral ischemia.…”

“…There are several reported cases of patients with Moyamoya disease becoming pregnant. [1][2][3][4][5][6][7][17][18][19][20] Careful monitoring of their blood pressure is essential to prevent intracranial complications during the pregnancy and parturition. The optimum blood pressure, however, is unknown.…”

“…
Moyamoya disease is a rare cause of intracranial hemorrhage during pregnancy. [1][2][3][4][5][6][7][8] More common causes include ruptured arterial aneurysms and arterio-venous malformations. 9 During pregnancy there is an increase in blood volume and cardiac output and later an acute rise in blood pressure during labour.
