Occult metastases from supraglottic laryngeal carcinoma

Abstract: One hundred and sixty-one patients with a supraglottic laryngeal carcinoma and clinically negative neck were studied. All patients had primary surgery and occult cervical node metastases were found in 18% (29/161). Local dissemination of tumour in the epilarynx did not influence the incidence of occult metastases, while the incidence of occult metastases increased with the degree of local spread in the supraglottis excluding the epilarynx. Ipsilateral occult metastases were more common (76%, 22/29), but both b… Show more

“…In our study females comprised almost one-third of the study population, and the mean age of them was not significantly different than for male patients as reported by others [20]. Women with laryngeal carcinoma generally do better than men [2,7,17,26]. The possible explanations of it involved among others less advanced disease and lower age at the time of diagnosis or more hygienic life style.…”

“…The best treatment for early supraglottic carcinoma has been a matter of debate for years mostly due to significant possibility of regional spread of disease [2,7]. Single modalities of surgery or radiotherapy are both options for such patients.…”

“…A 10% rate of occult metastases has been consistently demonstrated in glottic recurrence patients who were N0 initially and at recurrence . In contrast, supraglottic cancers have been reported to present with occult nodes in up to 43% of cases, and transglottic cancers with even higher rates…”

“…The reported highest density of lymphatic collectors in the triangle between the epiglottis, the false vocal cord, and the aryepiglottic fold28, 30 could explain the high prevalence of nodal metastases in supraglottic carcinoma with no statistically significant difference between marginal and vestibular tumors (47% vs 38%). No significant difference in occult metastases has also been reported from these subsites 36…”
