Octreotide improves early dumping syndrome potentially through incretins: a case report

Abstract: Dumping synDrome, or rapid gastric emptying, is a serious complication that may occur after gastric surgery in approximately 5-10% of patients, although it may occur in approximately 20% after vagotomy with pyloroplasty and up to 50% after esophagectomy [1][2][3]. “…In this study, depot octreotide (20 mg IM) significantly reduced diarrhea and requirement for PN in the former but not the latter group [64]. A subset of patients with diarrhea and/or hypoglycemia from dumping syndrome seem to uniquely benefit from octreotide therapy as it likely decreases incretin secretion (GLP-1 and GIP) and hormonally corrects this foregut disorder [65]. It is noted though that the use of long-term octreotide may increase the risk of biliary disease in this group of patients, who are already predisposed [66] and may decrease postresectional intestinal adaptation [67].…”
