Ocular surface squamous neoplasia in HIV-infected patients: current perspectives

Abstract: Ocular surface squamous neoplasia (OSSN) refers to a spectrum of conjunctival and corneal epithelial tumors including dysplasia, carcinoma in situ, and invasive carcinoma. In this article, we discuss the current perspectives of OSSN associated with HIV infection, focusing mainly on the epidemiology, pathophysiology, clinical manifestations, diagnosis, and treatment of these tumors in patients with HIV. Upsurge in the incidence of OSSN with the HIV pandemic most severely affected sub-Saharan Africa, due to asso… Show more

“…However, understaging and misdiagnosis of this condition often lead to a preventable loss of vision and the need for more aggressive treatments. HIV infection or predisposing conditions, such as xeroderma pigmentosum or atopic conjunctivitis, should be ruled out in all patients with a diagnosis of OSSN and atypical presentation (i.e., young age, bilateral or multifocal tumors, and history of rapid tumor growth) [ 24 ].…”

“…A role for HPV has also been proposed but not proven. 55 Given this combination, it is not surprising that there has been a rapid increase in the incidence of OSSN in SSA. 56 - 58 As OSSN is one of the first presenting signs of HIV/AIDS in upwards of 50% of cases, 55 there have been increased efforts to screen for and treat the condition.…”

“…Men in their 6th decade of life are commonly affected, but these lesions can also occur in younger individuals, especially when associated with xeroderma pigmentosum (XP) or human immunodeficiency virus (HIV) [811]. Studies in Africa report an increased incidence of OSSN in individuals with HIV compared to non-infected individuals [9, 12]. Furthermore, the biology of OSSN is more aggressive in African American individuals with HIV, with more frequent invasion into the orbit than is seen in individuals with HIV in western countries [13].…”
