Background: Postoperative atrial fibrillation (AF) is a common arrhythmia that occurs after coronary artery bypass grafting (CABG). 3% and 19.3% of CABG and OPCAB patients, respectively (p = 0.3). The peak incidence of arrhythmia was observed between the second and third postoperative day in both CABG and OPCAB patients (36% and 41%, respectively). Patient's age and history of hypertension were significant predictors of postoperative AF (OR 1.38, p = 0.0002; and OR 1.38, p = 0.008, respectively). Patients who developed AF vs. without AF had significantly higher rates of complications such as death (3.1% vs. 1.2%, p = 0.01), reoperation (5.2% vs. 2.8%, p = 0.02), and the need to utilize intra-aortic balloon pump (IABP) (6.8% vs. 3.4%, p = 0.002). Use of IABP and reoperation were significant perioperative predictors of the arrhythmia (OR 2.1, p = 0.003; and OR 1.9, p = 0.02, respectively). AF was also associated with a prolonged stay in an intensive care unit (72.5 ± 78.8 for patients with AF vs. 34.6 ± 25.2 for patients with sinus rhythm, p = 0.000001). Conclusions:In patients undergoing CABG, postoperative AF is a common arrhythmia independent of the type of surgical procedure. (Cardiol J 2016; 23, 5: 518-523)

Background: The new onset of atrial fibrillation (NOAF) can be occurred after coronary artery bypass grafting (CABG) surgery. NOAF can be occurred because of postoperative severe hemodynamic instability, long duration of ICU and hospital staying time, morbidity and mortality. Our aim of this study was to investigate whether N-terminal pro-brain natriuretic peptide (NT-proBNP) is a cause of NOAF after CABG. Methods: Forty CABG patients were enrolled for this study. Twenty patients operated on cardiopulmonary bypass (Group I; n: 20). The remaining patients have operated using a beating heart technique (Group II; n: 20). The NT-proBNP levels were calculated preand postoperatively. Results: High rate of NOAF was detected in group I patients (P < 0.05). In both groups, the NT-proBNP levels were low with sinus rhythm. The NT-proBNP blood levels in different times (T1, T2, T3, T4) among Group I were found higher than Group II. Conclusions: Our research demonstrated that there was a strongly close relationship between blood NT-proBNP levels' and atrial fibrillation occurrence in CABG patients. According to our results to detect postoperative AF development in early time and its treatment, blood NT-proBNP levels can be calculated in those patients for reducing morbidity and mortality due to AF.

Amaç: Bu çalışmada atriyal fibrilasyon (AF) etyolojisinde kardiyopulmoner baypas ve kardiyak arreste yönelik işlemlerin etken olup olmadığı araştırıldı. Ça lış ma pla nı: Ocak 2006 -Ocak 2007 tarihleri arasında kliniğimizde aynı cerrahi ekip tarafından elektif koroner arter baypas greft (KABG) cerrahisi uygulanan ardışık 449 hasta retrospektif olarak incelendi. Hastalar iki gruba ayrıldı. Birinci grupta atan kalp işlemi ile koroner revaskülarizasyon yapılan 86 hasta (58 erkek, 28 kadın), ikinci grupta ise kardiyopulmoner baypas ile revaskülarizasyon yapılan 363 hasta (261 erkek, 102 kadın) bulunuyordu. Her iki gruptaki ameliyat sonrası AF insidansı ve bazı ameliyat sırası değişkenler incelendi ve karşılaştırıldı. Bul gu lar: Ameliyat sonrası AF insidansı, kardiyopulmoner baypas grubunda %21.4; atan kalp işlemi uygulanan grupta %3.4 idi. Kardiyopulmoner baypas kullanımından kaçınılmasının ameliyat sonrası AF insidansını istatistiksel olarak anlamlı bir şekilde azalttığı görüldü. So nuç: Kardiyopulmoner baypas kullanılmadan yapılan miyokard revaskülarizasyonu sonrası AF insidansı belirgin olarak daha düşüktür. Background:In this study, we aimed to investigate the possible effects of cardiopulmonary bypass and procedures for cardiac arrest in the etiology of atrial fibrillation (AF). Methods: Between January 2006 and January 2007, consecutive 449 patients who underwent elective coronary artery bypass graft (CABG) surgery in our clinic were retrospectively analyzed. The patients were divided in two groups. The first group included 86 patients (58 males, 28 females) who underwent off-pump coronary revascularization, whereas the second group included 363 patients (261 males, 102 females) who underwent coronary revascularization with cardiopulmonary bypass. The incidence of postoperative AF and some perioperative variables in both groups were studied and compared. Results: The incidence of postoperative AF was 21.4% in cardiopulmonary bypass group and 3.4% in off-pump group. Avoiding cardiopulmonary bypass resulted in a statistically significantly reduction in the incidence of postoperative AF. Conclusion: The incidence of AF following myocardial revascularization without cardiopulmonary bypass is considerably lower. Our study results suggest that offpump procedure is very effective in reducing the incidence of postoperative AF.
