Inguinal omental fibromatosis is a rare disease entity that could mimic incarcerated inguinal hernia clinically. In the present study, we review the incidence of inguinal omental fibromatosis at our centre. From 1 January1996 to 30 June 2016, all hernia operations performed in all the affiliated hospitals of the University of Hong Kong were reviewed retrospectively; data were retrieved from patient records. A total of 7039 hernia operations were carried out during the period, in which 564 were incarcerated or strangulated hernia operations, and two were omental fibromatosis, accounting for an incidence of 0.028 per cent of groin explorations. In the present study, we report the second case at our centre. A 26‐year‐old man was admitted with a history of reducible right groin mass from birth. The mass had become irreducible for 2 months. The patient had no symptoms of bowel obstruction. On palpation, a firm mass was found in the right groin extending to the right scrotum, and could not be reduced completely. Bilateral testes in the scrotum were palpable. Computed tomography scan of pelvic cavity showed that there was a herniated omentum in the right scrotum. The omental mass was completely resected and free‐tension repair was performed. The histopathological examination revealed that the tumour consisted of spindle‐shaped cells that were consistent with fibromatosis. Inguinal omental fibromatosis is rare and could present as incarcerated hernia clinically. Fibromatosis could be part of a syndromal disease presentation, such as Gardner syndrome. Recurrence is higher than in sporadic cases, despite radical surgery.

A 68-year-old man, without a family history of cancer, was treated for primary cutaneous melanoma of the scalp. Two years later, a right lateral cervical lymph recurrence was observed and he was treated with lymphadenectomy and adjuvant nivolumab for 1 year. Four years from the initial melanoma diagnosis, a computer tomography scan showed a solid nodular lesion of 26 × 40 × 75 mm inside the previously known inguinoscrotal hernia. A new recurrence of melanoma was the most probable diagnosis and a right inguinal hernioplasty was performed. Notably, the histopathological examination revealed a mesenteric fibromatosis with the typical immunohistochemical pattern (strong nuclear staining of β-catenin). Interestingly, this represents the first case of a patient with a mesenteric desmoid tumour presenting as an inguinal hernia masking a cutaneous melanoma recurrence.

Clinical Characteristics of Incarcerated Inguinal Hernias of the Greater Omentum

Purpose: Incarcerated inguinal hernia may have very unusual hernial sac contents. In this study we aim to present clinical characteristics about incarcerated inguinal hernia of the greater omentum. Methods: We retrospectively analyzed hospital records of 643 adult patients who underwent inguinal hernia repair from November 2001 to January 2009. The age, sex, location, type, symptom duration, body mass index (BMI), comorbidity, type of anesthesia, operative time, postoperative hospital stay and complications were reviewed. Results: Of the 643 cases, 18 cases were omental incarcerated inguinal hernia group (O group). Seventeen in 18 cases of O group were male patient (NS). In O group, 16 cases were indirect type and 2 cases were femoral type (P=0.021). Mean age was younger in O group than in non-omental incarcerated inguinal hernia group (N group) (P=0.017). Duration of symptom was longer (P=0.013), and body mass index (BMI) was larger in O group (P=0.042). Operative time was longer in O group than in N group (P=0.006). Conclusion:The patients with omental incarcerated inguinal hernia mostly had long durations of protruding mass without pain. In these patients, operation was relatively difficult due to severe adhesion between the hernial sac and greater omentum. In addition, operative time took approximately 10 minutes longer. Therefore, general and spinal anesthesia is more feasible than local anesthesia.
