The delivery of therapies to the cochlea is notoriously challenging. It is an organ protected by a number of barriers that need to be overcome in the drug delivery process. Additionally, there are multiple sites of possible damage within the cochlea. Despite the many potential sites of damage, acquired otologic insults preferentially damage a single location. While progress has been made in techniques for inner ear drug delivery, the current techniques remain non-specific and our ability to deliver therapies in a cellspecific manner are limited. Fortunately, there are proteins specific to various cell-types within the cochlea (e.g., hair cells, spiral ganglion cells, stria vascularis) that function as biomarkers of site-specific damage. These protein biomarkers have potential to serve as targets for cell-specific inner ear drug delivery. In this manuscript, we review the concept of biomarkers and targetedinner ear drug delivery and the well-characterized protein biomarkers within each of the locations of interest within the cochlea. Our review will focus on targeted drug delivery in the setting of acquired otologic insults (e.g., ototoxicity, noise-induce hearing loss). The goal is not to discuss therapies to treat acquired otologic insults, rather, to establish potential concepts of how to deliver therapies in a targeted, cell-specific manner. Based on our review, it is clear that future of inner ear drug delivery is a discipline filled with potential that will require collaborative efforts among clinicians and scientists to optimize treatment of otologic insults.

Introducción La hipoacusia neurosensorial súbita es una emergencia otorrinolaringológica descrita desde 1944 y que continúa siendo un gran enigma. El objetivo es resumir e identificar los aspectos relevantes, avances y futuras perspectivas acerca del tema Metodología: Se realizó una revisión de literatura usando los motores de búsqueda de MEDLINE y Google académico de los últimos 5 años. Resultados: Es de causa idiopática en el 85% a 90%, con presentación bilateral en 0,4 a 3,4%, se han planteado numerosas teorías etiológicas entre ellas: vascular, inmunológica, estrés oxidativo, autoinmune, fístula perilinfática, traumáticas, viral, tumoral,entre otros, y en relación a ellas, numerosas líneas de tratamiento sin evidencia concluyente hasta el momento. Discusión: Frente a este cuadro clínico, se debe actuar inmediatamente; en el diagnóstico es importante la diferenciación con hipoacusia secundaria, la detección de signos de alarma, la forma de presentación y la severidad de la hipoacusia. No existe al momento un consenso global sobre su manejo, sin embargo, se recomienda iniciar con corticoterapia de manera inmediata, El uso de otras terapias aún se encuentra en debate, parecería tener evidencia prometedora el uso adyuvante con corticoides de vasodilatadores (PGE1), calcio antagonistas y acupuntura. La terapia de rescate parecería ser prometedora con el uso de oxígeno hiperbárico y corticoides intratimpánicos. Se han planteado otras terapias a futuro, buscando la estimulación de la regeneración de cilios celulares, o el reemplazo con células madre, sin embargo, estas terapias son conceptuales y aun no se han comprobado en estudios experimentales.

Acquired sensorineural hearing loss (SNHL), including age-related hearing loss (ARHL), noise-induced hearing loss (NIHL), drug-induced hearing loss (DIHL) and sudden sensorineural hearing loss (SSHL), is one of the most common sensory deficits in humans. Several studies have reported that antioxidant gene glutathione s-transferase M1 and T1 (GST M1 and T1) polymorphisms have a close relationship with the susceptibility to acquired SNHL, but other articles have reported opposite results. This meta-analysis aims to identify whether an association exists between GST M1 and T1 polymorphisms and the susceptibility to acquired SNHL. Seventeen independent studies containing 1749 cases and 2018 controls were included. According to the I2 value of the heterogeneity test, random-effects model was selected to calculate the pooled odds ratios (ORs) with their 95% confidence intervals (95% CIs) and p values. The pooled ORs (95% CI, p-value) of GST M1 and T1 were 1.186(0.955–1.473, p = 0.122) and 1.107(0.841–1.458, p = 1.467), respectively. In addition, subgroup analyses according to the type of SNHL and ethnicity showed no relationship between GST M1 and T1 polymorphisms and the susceptibility to acquired SNHL. Our results suggest that no significant relationship was found between GST M1 and T1 polymorphisms and the susceptibility to acquired SNHL.
