One Lesion, One Virus: Individual Components of High-Grade Anal Intraepithelial Neoplasia in HIV-Positive Men Contain a Single HPV Type

Abstract: Individual components of high-grade AIN are caused by single HPV types (the so-called one lesion, one virus concept). HPV16 is causative in <50% of cases. Anal swabs are not useful for detecting lesion-specific HPV types.

“…Twenty‐two percent of AIN 2/3 cases harbored HPV DNA from multiple HPV types compared to 7.3% in anal cancers. This decrease in multiplicity of infection with neoplastic disease progression has been also described for CIN 2/3 (cervical intraepithelial neoplasia) and cervical cancer, and may reflect the concept of clonal development of invasive neoplasia resulting from persistent infection with a single HPV type …”

“…This decrease in multiplicity of infection with neoplastic disease progression has been also described for CIN 2/3 (cervical intraepithelial neoplasia) and cervical cancer, 30 and may reflect the concept of clonal development of invasive neoplasia resulting from persistent infection with a single HPV type. 31,32 The accumulated knowledge on the natural history of anal cancer closely resembles that of cervical cancer. Both types of tumors share similar transmission mechanisms for HPVs as etiological agents, are modulated by similar risk factors and seem to arise from the same type of cells occurring at a transition zone between columnar-glandular and stratified epithelia.…”

“…Previous studies used HPV polymerase chain reaction (PCR) on whole-tissue sections (WTS) to identify HPV types in intra-anal warts, but detection of HPV DNA does not necessarily mean that it is causally related to the concurrent lesion, especially when multiple HPV types are observed. 14,15 We hypothesized that HGAIN regions in intra-anal warts are caused by hrHPV types. Laser capture microdissection (LCM) combined with sensitive and type-specific PCR has proved to be a reliable method to determine the lesion-specific HPV genotype in cervical intraepithelial neoplasia, vulvar intraepithelial neoplasia and in AIN (the one lesion, one virus concept).…”

[15][16][17][18] Therefore, in this study we used LCM to determine the causative HPV type of low-and high-grade dysplastic areas in intra-anal warts obtained in HIV-positive MSM.…”

“…Condyloma acuminatum is most often associated with HPV 6 or 11, but in HIV-infected individuals, a high proportion may be co-infected with HPV 16 or 18. Rather than refl ect co-infection of individual cells with more than one HPV type (Richel et al 2014 ), these probably refl ect separate foci of clinically subtle HSIL, with HPV 6 or 11 causing the condyloma and HPV 16 causing the HSIL. HSIL is believed to be the true cancer precursor of anal cancer, similar to the role of cervical HSIL as the precursor to cervical cancer (Berry et al 2014 ).…”
