One-Stage Immediate Alloplastic Breast Reconstruction in Large and Ptotic Breasts: An Institutional Algorithm

Abstract: Immediate implant-based breast reconstruction in patients with large and ptotic breasts may be challenging due to skin redundancy. The use of a reduction mammoplasty pattern for the mastectomy skin excision has proven to be a reliable option for these patients as it allows for a better shape, projection, and symmetrization. This approach has been described in the literature for both one- and two-stage reconstruction with either sub- or pre-pectoral reconstruction with an acellular dermal matrix (ADM) or non-bi… Show more

“…This fact may also explain the limited number of patients that resulted eligible for the described technique over a sixteen-month period in a hub center for mammary carcinoma treatment. In case of mastectomy skin flap with a thickness less than 1 cm or pectoral fascia removal, the Authors opt for a subpectoral reconstruction or a pre-pectoral reconstruction with ADM, respectively, according to the Institutional Algorithm [ 23 ]. Contralateral reduction mammoplasty is most of the time required when the patient is eligible for unilateral skin reducing mastectomy.…”

“…The surgical technique has been conceived for women with large and ptotic breasts who require skin or nipple-sparing mastectomy with reduction of the skin and are not willing or are not eligible for autologous tissue reconstruction. An institutional algorithm [ 23 ] was applied for patient selection.…”

“… 41 Single-stage breast reconstruction, on the other hand, involves placing a permanent implant during the initial surgery. 42 This approach is typically used when there is enough remaining breast tissue to cover the implant and provide a natural-looking result.…”

“…14,15 Mastectomy flap thickness and vascularization were fundamental criteria for prepectoral reconstruction, and after confirmation of good perfusion and flap viability, assessment of internal dimensions determined the final implant size. [16][17][18][19][20][21] Achieving a complete fit between the implant and the periprosthetic space was crucial. 22 Implant size considerations aimed to prevent dead space, seroma formation, and complications like rippling, wrinkling, and skin necrosis.…”
