Only physical aspects of quality of life are significantly improved by bilateral subthalamic stimulation in Parkinson’s disease

Abstract: Using clinician's based rating scale, bilateral STN DBS showed significant improvement in PD patients at 12 month follow up. However, using patient's self-assessment scales, the clinical benefit of STN DBS was more subtle: physical items of QoL significantly improved, whereas mental items such as emotional well-being, social support, cognition and communication showed no improvement. Our results are suggestive of a dissociation of motor and non-motor symptoms control after bilateral STN DBS in PD patients.

“…Quality of life improves after STN DBS, 56,[63][64][65][66][67] and this improvement correlates with motor benefit. 65 In support of this finding, the physical markers for quality of life are reported to improve significantly, but other aspects such as mental, emotional well-being, social support, cognition, and communication show no improvement.…”

“…STN stimulation could have a direct effect on affective disorders through its limbic connections. Our group has already demonstrated that some affective disorder can follow STN-DBS in PD, particularly in the emotional domain [5,10]. Current estimates of the prevalence of apathy in Parkinson's Disease vary between 16.5% [1] to 42% [41], depending upon the instrument used for assessment and the samples examined.…”

“…However, improvement in quality of life after bilateral STN stimulation can be related to only physical aspects. 42 Neuropsychological decline was not associated with improvement in motor scores. This implies that patients who did not show much improvement in motor functioning were not necessarily the ones who showed cognitive decline.…”
