Onset of Depression in Elderly Persons After Hip Fracture: Implications for Prevention and Early Intervention of Late‐Life Depression

Abstract: The onset of MDD is common after hip fracture, and the greatest period of risk is immediately after the fracture. Individuals with clinical evidence of apathy are at high risk for developing MDD, and evaluation and close follow-up of such individuals is warranted. However, further research is needed to examine other candidate variables (e.g., clinical measures or biomarkers) to model adequately the risk for MDD after hip fracture and other disabling medical events.

“…66 In the period after a hip fracture, 15% of older adults meet criteria for major depression, and half will have elevated depressive symptoms. 67 In some individuals, a high level of distress immediately after the injury improves over time. 68 In a smaller proportion, depressive symptoms seem to arise as their recovery plateaus and they are faced with enduring deficits.…”

“…71 Age, medical comorbidity, level of disability before and after the fracture, apathy, delirium, cognitive impairment, and poor social support are all risk factors for the development of depression after hip fracture. 67 Use of an antidepressant at baseline, perhaps an indicator of a history of depression, is also a risk factor. 67 Gene polymorphisms in serotonin transporter and 5HT-1A receptor, markers of vulnerability to stress, have also been implicated in the development of depression and impaired function after a hip fracture.…”

“…Additionally, 11 domain-specific measures were reviewed: three measures of emotional wellbeing -Geriatric Depression Scale (GDS) [42], Hamilton Rating Scale for Depression (HAM-D) [43] and the Zung Depression Inventory (ZungDI) [44]; and eight measures of (instrumental) activities of daily living (I/ADL): Barthel Index (BI) [45], Modified-Barthel Index [46], Functional Activities Index (FAI) [47], Functional Independence Measure (FIM) [48], Functional Status Questionnaire (FSQ) [49], Lawton IADL scale (Lawton-IADL) [50], Katz Index of Independence in ADL (Katz ADL) [51] and the OARS Multi-dimensional Functional Assessment Questionnaire (OMFAQ) [52]. One of these measures (Katz ADL) is clinician-completed but was included in the review due to its wide-spread use in this patient population.…”

“…Isso ocorre provavelmente pelo efeito estressante associado à fratura incluindo sequelas, dor, imobilização, hospitalização e cirurgias. Um estudo prospectivo com 126 idosos revelou que o período de maior risco para surgimento da depressão foi nas primeiras 10 semanas após a fratura 15 .…”
