Open surgical and endovascular treatment of superior vena cava syndrome caused by nonmalignant disease

Abstract: Surgical treatment of benign SVC syndrome is effective over the long term, with secondary endovascular interventions to maintain graft patency. Straight spiral saphenous vein graft remains the conduit of choice for surgical reconstruction, with results superior to those with bifurcated vein and ePTFE. Endovascular treatment is effective over the short term, with frequent need for repeat interventions. It does not adversely affect future open surgical reconstruction and may prove to be a reasonable primary inte… Show more

“…Grafting of the superior vena cava grafts using autogenous spiral saphenous vein, autogenous superficial femoral vein or stents (PTFE or GoreTex®) is the treatment of choice for superior vena cava syndrome of benign etiology. (12) Reconstruction with autogenous spiral saphenous vein achieves excellent clinical results and a perviousness index of 80% in the long run. (12) The superficial femoral vein makes a good graft, but has been less frequently used due to the occurrence of thrombosis and distal venous vascular insufficiency at the donor site.…”

“…Use of long-term anticoagulation therapy is controversial. Endovascular intervention is needed if medical management fails or if there is recurrence of the syndrome [1,3,4]. “…However, the availability of new flexible intravascular stents, allowing access even to tortuous vessels, provides a new therapeutic approach for patients with vascular problems in diverse specialties. There have been significant declines in the use of SV grafting for the treatment of coronary ostioplasty,12 cerebrovascular disorders,13 superior vena cava syndrome,14 and visceral revascularization15 due to the introduction of angioplasty and stenting techniques. The percutaneous transluminal angioplasty and stenting, however, may develop complications or recurrent symptoms, which may eventually warrant surgical intervention 15.…”
