Open versus laparoscopically-assisted oesophagectomy for cancer: a multicentre randomised controlled phase III trial - the MIRO trial

Abstract: BackgroundOpen transthoracic oesophagectomy is the standard treatment for infracarinal resectable oesophageal carcinomas, although it is associated with high mortality and morbidity rates of 2 to 10% and 30 to 50%, respectively, for both the abdominal and thoracic approaches. The worldwide popularity of laparoscopic techniques is based on promising results, including lower postoperative morbidity rates, which are related to the reduced postoperative trauma. We hypothesise that the laparoscopic abdominal approa… Show more

“…Hence, studies on resection techniques in esophageal cancer surgery focus mainly on a reduction of morbidity, particularly respiratory complications and pneumonia. It is remarkable that these studies use different definitions of pneumonia [18,21] or fail to provide a definition altogether [22,23,24,25,26]. Consequently, the reported pneumonia rates vary widely.…”

“…These limitations pave the way for the occurrence of unknown interactions and confounding variables. Currently, there are two diferent clinical trials comparing E to MIE patients, namely the French MIRO trial [52] and the British ROMIRO trail [53]. Still, the study that we conducted is the most extensive comparative efectiveness research of MIE assessing long-term survival.…”

“…The comparison of laparoscopic approach and open for the abdominal time is the subject of a French Study (MIRO-Trial), which shows a clear benefit for this hybrid minimally invasive approach [18]. Table 2 resumes the treatment for junction tumor according to the Siewert Classification.…”
