The renewables sector and particularly offshore wind energy is a fast developing industry over the last few years. Especially activities related to the Installation, Operation and Maintenance (O&M) of offshore wind turbines becomes a challenging task with inherent risks. This paper assesses the risks related to the above stages of a wind farm lifecycle using the FMECA (Failure Mode, Effects and Criticality Analysis) and HAZID (Hazard Identification) methods. All works, from installation to O&M are considered together with the wind turbine main components. An integrated risk analysis methodology is presented addressing personnel Safety (S), Environmental impact (E), Asset integrity (A) and Operation (O). The above is supplemented by a cost analysis with the aid of BBN (Bayesian Belief Networks) method in order to assist the decision making process related to installation and O&M tasks. All major risks and critical wind turbine components are identified as well as measures are suggested in order to prevent or mitigate them. Moreover, inspection and maintenance plans are elaborated in general for the mentioned activities.

Several plants in Brazil have mature assets or use equipment that comes from decommissioned facilities. In addition, the culture of process safety in Brazil is an under‐researched topic that has only recently begun to attract corporate attention. Since catastrophic accidents are not very common, any accident related to the process can result in very large losses in terms of damage to equipment and loss of human and social capital. In mature process plants, these issues are even more sensitive because their organization may not yet have developed an effective integrity management program. Nevertheless, it is a proven fact that the assets of these plants are often nearing the end of their residual lifetime. Therefore, the present work aims to develop a high‐level checklist to evaluate the mechanical integrity and identify possible failures in the integrity management of vessels and tanks, based in good engineering practices, Regulatory Standard 13 (NR 13), API 579‐1/ASME FFS‐1, API RP 580, API RP 581, HSE (Health and Safety Executive) requirements, and other relevant standards as references. The checklist ts divided into six sections, namely (1) Brazilian Regulatory Standard 13 (NR 13); (2) Design and manufacturing; (3) Operation; (4) Management systems; (5) Deterioration mechanism; and (6) Risk‐based inspection, as the most relevant criteria for evaluating the integrity of mature pressure vessels and tanks.

Purpose -The purpose of this paper is to propose a risk-based approach for spare parts demand forecast and spare parts inventory management for effective allocation of limited resources. Design/methodology/approach -To meet the availability target and to reduce downtime, process facilities usually maintain inventory of spare parts. The maintaining of non-optimized spare parts inventory claims more idle investment. Even if it is optimized, lack of attention towards the critical equipment spares could threaten the availability of the plant. This paper deals with the various facets of spare parts inventory management, mainly risk-based spare parts criticality ranking, forecasting, and effective risk reduction through strategic procurement policy to ensure spare parts availability. A risk-based approach is presented that helps managing spare parts requirement effectively considering the criticality of the components. It also helps ensuring the adequacy of spare parts inventory on the basis of equipment criticality and dormant failure without compromising the overall availability of the plant. Findings -The paper proposes a risk-based approach that used conjugate distribution technique with the capability to incorporate historical failure rate as well as expert judgment to estimate the future spare demand through posterior demand distribution. The approach continuously updates the prior distribution with most recent observation to give posterior demand distribution. Hence the approach is unique in its kind. Practical implications -Appropriate spare parts unavailability could have great impact on process operation and result in costly downtime of the plant. Following proposed approach the availability target can be achieved in process industry having limited maintenance resources, by forecasting spare parts demand precisely and maintaining inventory in good condition. Originality/value -Adopting the approach proposed in the paper, risk level can be minimized and plant availability can be maximized within the financial constraint. The resources are allocated to the most critical components and thereby increased availability, and reduce risk.
