Opioid‐related mortality and filled prescriptions for buprenorphine and methadone

Abstract: This study showed that most fatalities were not related to filled prescriptions of maintenance drugs, and a substantial illicit use of buprenorphine and methadone resulting in deaths was revealed. To prevent opioid toxicity deaths it is important to make efforts not only to reduce drug diversion from maintenance programs, but also to improve the control of drug trafficking and other illegal sources.

“…However, about 70% of the decedents in our study were enrolled in a drug treatment facility at the time of death and more than 60% were receiving MMT. These results differ from the findings of other studies where the majority of those with methadone-related deaths were not receiving MMT at time of death [4,19,20,33,34] . Although elevated mortality risk in MMT is usually observed during the induction period, only about 5% of MMT deaths took place during the first 4 weeks of treatment in the current study [35,36] .…”

“…Previous studies have primarily investigated psychoactive prescription drugs as a group (Hall et al, 2008;Rönkä el al., 2015), or focused on a separate drug class, such as opioids (Fischer et al, 2014;Häkkinen et al, 2014;Paulozzi et al, 2009;Wikner et al, 2014).…”

“…The second strand of research is looking at the negative consequences of illicit use, mainly in the form of methadoneand buprenorphine-related mortality among users not in treatment (Auriacombe, Fatseas, Dubernet, Daulouede, & Tignol, 2004;Fugelstad, Johansson, & Thiblin, 2010;Madden & Shapiro, 2011;Mégarbane et al, 2006;Morgan, Griffiths, & Hickman, 2006;Seldén, Ahlner, Druid, & Kronstrand, 2012;Seymour et al, 2003;Strang et al, 2010;Wikner et al, 2014). The majority of such fatalities are caused by polyintoxication involving a broad array of different substances, often sedatives and alcohol (Fugelstad et al, 2010;Mégarbane et al, 2006;Seldén et al, 2012;Wikner et al, 2014).…”

Such deaths have made up a significant percentage of the opioidrelated mortality in some countries, not least Sweden; since 2011 the combined number of methadone-and buprenorphine-related fatalities have exceeded heroin-related deaths.…”
