Optic neuritis classification in 2021

Abstract: Optic neuritis (ON) can be associated with inflammatory disease of the central nervous system or can be isolated, with or without relapse. It can also be associated with infectious or systemic disease. These multiple associations based on a variety of clinical, radiological, and biological criteria that have changed over time have led to overlapping phenotypes: a single ON case can be classified in several ways simultaneously or over time. As early, intensive treatment is often required, its diagnosis should b… Show more

“…In the Optic Neuritis Treatment Trial, even among those presenting with count fingers vision or worse at presentation, only 8% had a final visual acuity of 6/60 or worse at last follow-up 1. In contrast, untreated optic neuritis associated with neuromyelitis optica spectrum disorder is associated with very severe vision loss, with about half having a final visual acuity of 6/60 or worse 2. Finally, in demyelinating disease and neuromyelitis optica spectrum disorder, there are usually associated white matter lesions elsewhere in the brain, not present here.…”

“…Isolated optic neuritis is often attributed to clinically isolated syndrome (CIS). CIS is an ambiguous term, and it is generally used for the pre‐MS clinical case (Ducloyer et al, 2021; Kale, 2016). A review article on the classification of optic neuritis was published by Ducloyer et al in 2021.…”

“…First, it is probable that in NMO several autoantibodies are involved in the pathophysiology. 57 In addition, AQP4-Ab autoreactivity is probably polyclonal in human disease. Thus, we decided, on purpose, to use IgG from NMO patients that reflect the diversity of the process and we cannot exclude that some alterations could be mediated by other antibodies in our models.…”
