Optical Coherence Tomography Predictors of Favorable Functional Response in Naïve Diabetic Macular Edema Eyes Treated with Dexamethasone Implants as a First-Line Agent

Abstract: Purpose. To evaluate efficacy and safety of intravitreal dexamethasone 0.7 mg implant in treatment-naïve DME patients and to assess the utility of OCT structural biomarkers as predictors of functional response after treatment. Methods. Thirty-nine eyes of 39 diabetic patients with center involving DME were enrolled. Best-corrected visual acuity (BCVA) and SS-OCT (DRI SS-OCT Triton, Topcon, Japan) to evaluate central retinal thickness (CRT), serous retinal detachment (SRD), intraretinal cysts (IRC), number of h… Show more

“…All our patients received anti-VEGF agents as first line therapy; therefore, the isolated effect of dexamethasone on OCT biomarkers such as EZ integrity was not evaluated. EZ integrity has been identified as a positive predictor for VA gain in patients with treatment naive or refractory DME who received dexamethasone implants [ 42 , 43 ]. Our findings support a positive correlation between EZ restoration and VA improvement.…”

“…Their study empathized the importance of structural biomarkers as predictors of favorable response and confirmed the efficacy and safety of intravitreal dexamethasone implant in treatment-naïve DME patients showing a better functional response in the presence of serous retinal detachment (SRD), the integrity of ellipsoid zone (EZ) and absence of vitreomacular alterations. 33 However, the structural damage of the retinal capillary wall occurs due to toxic effects of metabolites, mainly, anomalous glucose metabolism, lipid peroxidation, and endothelial cell injury- secondary to inflammatory leukostasis owing to the resultant response of increased secretion of VEGF. The hike in VEGF follows the consequent appearance and progression of DR. 10 This study demonstrated the efficacy and cost-effectiveness of anti-VEGF agents aided by laser therapy to treat DME in the Indian population.…”
