Optimization of electromyographic endotracheal tube electrode position by UEScope for monitored thyroidectomy

Abstract: Objective: Proper position of an electromyographic (EMG) endotracheal tube within the larynx plays a key role in functional electrophysiologic intraoperative neural monitoring (IONM) in thyroid surgery. The purpose of this study was to determine the feasibility of a portable video-assisted intubation device (UEScope) to verify the optimal placement of an EMG tube. Methods: A retrospective study enrolled 40 consecutive patients who underwent monitored thyroidectomies. After positioning the patient for surgery, a… Show more

“…Namely, the placement and position of such an EMG-ET tube may be difficult and need to be checked, adjusted, and optimized. Such a need can be met by using either DL/FOB [36][37][38][39], VL [36,[40][41][42][43], and an optic intubating stylet [36,44,45]. In this brief report, we demonstrate the routine application of combined styletubation with videolaryngoscopy for tracheal intubation in patients undergoing a thyroidectomy with intraoperative IONM.…”

“…Both good contact of surface electrodes (13)(14)(15)(16)(17)(18)(19) and adequate reversal of neuromuscular blockade (NMB) (20)(21)(22)(23)(24)(25)(26)(27) are prerequisites for successful IONM of the RLN. For adequate relaxation to facilitate tracheal intubation and surgery, a profound or deep NMB is desirable.…”
