Our objective was to compare the outcomes of dual kidney transplanataion (DKT) to single kidney transplantation (SKT) performed with grafts from expanded criteria donors (ECD) in recipients ≥65 years, focusing on surgical complications. All kidney transplantations (KT) performed between 2006 and 2014 in our institution were analysed. DKT was indicated according to the criteria of the French national Agence de la Biomedecine. Thirty-nine DKT and 155 SKT were included, with a median follow-up of 36 and 26.5 months, respectively. The rate of early surgical revisions was not significantly higher after DKT (23.1% vs 15.5% (P = 0.2593)) but more venous graft thromboses (12.8% vs 3.2% (P = 0.02)) were reported. The glomerular filtration rate (GFR) 24 months after KT was significantly higher after DKT (45.0 ± 16.3 vs 39.8 ± 13.8 ml/min/1.73m ; P = 0.04) and allowed shorter waiting time without a significant increased risk of surgical revision, excepted for venous graft thrombosis, more frequent after DKT. Graft survivals were not significantly different and GFR was higher after DKT. DKT seems to remain an appropriate strategy to address the growing graft shortage in elderly patients.

Objectives: We report the first en bloc adult deceased-donor renal transplant at our center. We had a an extended criteria, female, deceased donor, who was a known hypertensive, hypothyroid, with a history of Hodgkin lymphoma that had been treated successfully 30 years prior without recurrence. Her cause of death was intracranial hemorrhage. In view of the raised creatinine level at the time of donation, the transplant team decided to accept the kidneys for dual transplant. We decided to use a technique that is used to recover pediatric en bloc kidneys in our adult deceased donor. Materials and Methods: The kidneys were recovered en bloc with the aorta and inferior vena cava. Meticulous back table work was required in ligating all the nonrenal tributaries of the donor aorta and inferior vena cava. We anastomosed the aorta to the common iliac artery in an oblique end-to-side fashion. The donor inferior vena cava was anastomosed to the external iliac vein in an end-to-side fashion. The distal ends of ureters were made into a common lumen with a side-to-side anastomosis, and this common ureter was implanted into the bladder. Results: There was no requirement for postoperative dialysis. The creatinine is at nadir of 88 μmol/L at 1 year of follow up. Conclusions: This is a method that can be attempted for implanting dual kidneys on one side. Kidneys with multiple vessels can be transplanted like a single kidney with a shorter cold ischemia/overall operative time and less fatigue for the surgeon.

The limiting factor in conventional quality assessments of transplanted organs, namely the invasiveness of tissue sample collection, has prompted much research on the field of transplantology to focus on the development of alternative evaluation methods of organ quality. In the present project, we undertake the challenge to address the need for a new analytical solution for graft quality assessments by using a novel metabolomic diagnostic protocol based on low-invasive solid-phase microextraction. Solid-phase microextraction probes of ca. 0.2 mm coated with 4 mm long mixed-mode extraction phase were inserted into rabbit kidneys immediately following euthanasia and after 2, 4, 6, and 21 h of preservation. Liquid chromatography-mass spectrometry analysis of the extracts was performed with the use of a reversed phase column and a Q-Exactive Focus mass spectrometer operated in positive ionization mode. Statistical analysis of significantly changing compounds revealed metabolic profile changes in kidneys induced by ischemia and oxidative stress as a function of the duration of cold storage.The most pronounced alterations were reflected in levels of essential amino acids and purine nucleosides. Our findings demonstrate that the proposed approach may be successfully used to monitor changes in the metabolic profile of organs over time of preservation.
