Optimization of Whole-Body CT Examinations of Polytrauma Patients in Comparison with the Current Diagnostic Reference Levels

Abstract: Purpose Evaluation of the dose values of a polytrauma whole-body CT examination used in clinical practice with regard to the 2016 updated diagnostic reference levels and reduction of the mean exposure levels using simple optimization steps. Materials and Methods In each case, 100 exposure values before and after dose optimization were compared with the old and new diagnostic reference levels. The grayscale values and the signal-to-noise ratio (SNR) were determined for the lung, the aortic arch and … Show more

“…Saudi Arabia has a high number of road traffic accidents, 2020 according to the Ministry of health statistics there are total injuries of 25,561 and 4,618 deaths, the Saudi health system is providing the emergency departments and trauma centers with very advanced high technical machines for imaging and scanning of trauma patients in which we need to make a proper prioritization and utilization of it especially the CT machines to avoid the disadvantages of unnecessary radiation [4].…”

“…Many trauma centers have clear justifications for performing WBCT which is shared between the physicians and radiologists [5], however, there is several patients seem to be matching with the criteria for WBCT but the final report is normal WBCT, this safety margin of error looks acceptable for the most of the physicians and health system as well [4].…”

“…Although the classic approach to major trauma with conventional imaging and selective scanning is still advocated in children [13,14], WBCT is increasingly used [12]. Various studies report reduction of radiation while maintaining adequate image quality in adult patients undergoing CT examinations [15][16][17]. Following the ALARA concept, continuous efforts to achieve the lowest radiation exposure possible is essential for any paediatric centre [18,19].…”

“…Viele der in der Literatur beschriebenen Vorteile für die Patientenversorgung z. B. durch Reduktion der Röhrenspannung, spektrale Aufhärtung mittels Zinnfilter oder Dual Energy erfordern einen hohen Ausbildungsgrad der Untersucher und eine genaue Kenntnis der Patientencharakteristiken [2][3][4][5]. Die Kontrastmitteldosis kann in Abhängigkeit von der Röhrenspannung und dem Körpergewicht individuell angepasst werden.…”

“…In Germany, computed tomography (CT) is the most frequently performed slice imaging technique, with growth of over 45 % between 2007 and 2016 [1]. Many of the advantages for patient care described in the literature, e. g. reduction of tube voltage, spectral hardening using tin filters or dual energy, require a high degree of training of the examiners and a precise knowledge of the patient's physical characteristics [2][3][4][5]. The contrast medium dose can be individually adjusted as a function of tube voltage and body weight.…”
