Optimizing Initial Chemotherapy for Metastatic Pancreatic Cancer

Abstract: The two combination chemotherapy regimens FOLFIRINOX and gemcitabine plus nab-paclitaxel represent major breakthroughs in the management of metastatic pancreatic cancer. Both regimens showed unprecedented survival advantage in the setting of front-line therapy. However, their application for treatment of patients in the community is challenging because of significant toxicities, thus limiting potential benefits to a narrow population of patients. Modifications to the dose intensity or schedule of those regimen… Show more

“…a potent anti-cancer effect for PC treatment by inducing cell apoptosis, inhibiting cell proliferation, and suppressing cell migration and invasion of PC cells in pre-clinical studies (21). However, whether curcumin has synergistic effects with either gemcitabine or docetaxel for the treatment of PC remains elusive.…”

“…Curcumin, a hydrophobic polyphenol extracted from dried rhizomes of turmeric, is a kind of traditional Chinese herbal medicine and a potential drug for cancer therapy ( 20 ). A growing body of evidence has demonstrated that curcumin has a potent anti-cancer effect for PC treatment by inducing cell apoptosis, inhibiting cell proliferation, and suppressing cell migration and invasion of PC cells in pre-clinical studies ( 21 ). However, whether curcumin has synergistic effects with either gemcitabine or docetaxel for the treatment of PC remains elusive.…”

“…However, despite this increased toxicity the FOLFIRINOX patients had a significantly longer time to deterioration in quality of life than the gemcitabine patients [Gourgou-Bourgade et al 2013]. Furthermore, since the adoption of this regimen into the clinic various institutional series have been published suggesting modifications to the regimen to increase its tolerability without reducing efficacy [Mahaseth et al 2013; Mantripragada et al 2016].…”

“…Recent combinations of chemotherapy and target therapy such as erlotinib plus Gem have achieved a significant, albeit modest, increase in OS vs Gem in a global study of locally advanced or MPC patients. 7 The phase III trial, Metastatic Pancreatic Adenocarcinoma Clinical Trial (MPACT), showed improved efficacy of Abraxane ® (nab-paclitaxel; Celgene, Boudry, Switzerland) plus Gem compared to Gem alone. An updated OS analysis showed a 2-month difference in the arm with nab-paclitaxel plus Gem vs Gem alone (median, 8.7 vs 6.6 months) and a total reduction in the risk of death of 28%, an improvement in survival at 1 year of 59% and an improvement in survival rates at 2 years of 12%.…”
