Oral Administration of Cannabis and Δ-9-tetrahydrocannabinol (THC) Preparations: A Systematic Review

Abstract: Background and objective: Changes in cannabis legalization regimes in several countries have influenced the diversification of cannabis use. There is an ever-increasing number of cannabis forms available, which are gaining popularity for both recreational and therapeutic use. From a therapeutic perspective, oral cannabis containing Δ-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) is a promising route of administration but there is still little information about its pharmacokinetics (PK) effects in humans. … Show more

“…The exact mechanism of BEP remains speculative, as it has previously been attributed to either defect in cardiac ion channels resulting in depolarization/repolarization or abnormal expression of neural crest cells [ 13 ]. Our patient demonstrated reversible BEP after ingestion of delta-8 THC, which raises concerns about the safety profile of delta-8 THC.…”

“…As represented in Figure 2D , there is a clear difference in the duration of neurocognitive impairment depending on the route of administration (smoked vs. sublingual spray vs. oils). Due to differences in absorption and metabolism, THC has a different onset and duration of action depending on where in the body it is administered ( 57 59 ). Cannabis oils may provide up to 8 h of symptom relief due to gradual absorption of THC from the gut combined with first pass metabolism conversion of THC to 11-OH-THC, another active compound, in the liver ( 30 , 58 ).…”

“…The major cannabinoids are substrates for numerous metabolic enzymes, such as cytochrome P450 metabolizing enzymes ( 6 ). The PK of oral THC shows marked variability, with differences between formulations, for example, higher variability in baked goods and oil forms ( 14 ). The preparation method significantly impacts the final product characteristics ( 15 ).…”
