Oral Antidiabetic Agents

Abstract: Type 2 diabetes mellitus is a progressive and complex disorder that is difficult to treat effectively in the long term. The majority of patients are overweight or obese at diagnosis and will be unable to achieve or sustain near normoglycaemia without oral antidiabetic agents; a sizeable proportion of patients will eventually require insulin therapy to maintain long-term glycaemic control, either as monotherapy or in conjunction with oral antidiabetic therapy. The frequent need for escalating therapy is held to… Show more

“…This causes a reduction in the rate of glucose absorption and consequently blunts the postprandial blood glucose and insulin levels. Thus alpha-glucosidase inhibitors may be an attractive therapeutic modality in type II diabetic patients (Bischoff, 1994;Jiang & Du, 2011;Krentz & Bailey, 2005;Lin et al, 2011;Oboh & Ademosun, 2011).…”

“…6) Traditional herbal medicine has been widely used for disease treatment and is recognized as an interesting alternative to conventional medicine. [7][8][9] The uncertainty about the efficacy and safety of the currently available oral hypoglycemic drugs has prompted a search for safer and more effective agents in the treatment of diabetes. 8) A wide variety of the traditional herbal remedies being used by diabetic patients, especially in the third world countries, 9) and may, therefore, represent new avenues in the search for alternative hypoglycemic drugs.…”

“…Thiazolidinediones are potent, synthetic ligands for peroxisome proliferator-activated receptor gamma (PPARg) that potentiate the action of insulin in peripheral tissues, and are used as orally active treatments for diabetes in humans (Krentz and Bailey 2005). Kushibiki et al (2001) administered TZD to steers injected with TNF-a to induce insulin resistance and observed a decrease in plasma concentrations of NEFA, insulin and glucagon, suggesting that administration of TZD ameliorated insulin action.…”
