Oral Frailty as a Risk Factor for Physical Frailty and Mortality in Community-Dwelling Elderly

Abstract: Accumulated poor oral status strongly predicted the onset of adverse health outcomes, including mortality among the community-dwelling elderly. Prevention of oral frailty at an earlier stage is essential for healthy aging.

“…Specifically, they defined "oral frailty" as the presence of at least three of the following, and "pre-oral frailty" as the presence of one or two of the following: (i) <20 (natural) teeth; (ii) decline in chewing ability; (iii) decline in oral diadochokinesis (rate of rapid syllable repetition); (iv) decline in tongue pressure; (v) subjective difficulty in swallowing ("cannot eat hard food"); and (vi) subjective difficulty in drinking. 23 Of the 16 dental and intraoral indices, these six indices were determined to be risk factors of physical frailty. Under these criteria, 16% of the study's participants were "orally frail" and 50% were "pre-orally frail" at baseline.…”

“…Recent epidemiological studies suggest that decreased masticatory ability in the elderly is a risk factor for frailty 1 During normal food intake, food is pulverised, sheared, and mixed with saliva and gradually transported towards the pharynx as a bolus to be swallowed. 3,4 In the oral processing of food, the state of food comminution in a limited number/times of masticatory cycles or by the duration of mastication constitutes masticatory efficiency.…”

“…Recent epidemiological studies suggest that decreased masticatory ability in the elderly is a risk factor for frailty and death . Most of these studies used subjective and qualitative measures to evaluate masticatory function on account of the limitations of time and the setting for assessing a large number of individuals.…”
