Oral health in Australian HIV patients since the advent of combination antiretroviral therapy

Abstract: Background: The introduction of combination antiretroviral therapy (cART) for the treatment of human immunodeficiency virus (HIV) has resulted in changes to the oral health of infected individuals. Little data are available describing prevalence and severity of oral manifestations in a post cART cohort of HIV positive patients. “…However, our review revealed that LGE, a lesion classified in the ECC criteria as strongly associated with HIV, was not commonly encountered or reported in the literature. acute necrotizing ulcerative gingivitis and acute necrotizing ulcerative periodontitis were more predominant in Africa, as they are associated with poor oral hygiene and less accessibility to the oral care system . The reporting of periodontitis in HIV patients increased over the decades, and its prevalence was highest in studies where participants were on ART.…”

“…In addition, it appears that some HIV‐periodontal diseases may recur even if good viral control is achieved, with co‐factors such as stress and smoking appearing to have a role in HIV‐periodontal disease exacerbation. A preliminary retrospective analysis which looked at the oral health and treatment needs of HIV‐positive patients identified that patients on cART still had significant oral health issues including HIV‐related periodontal disease, oral candidiasis, angular cheilitis, HPV papillomas, traumatic ulcerations and xerostomia.…”

“…Interestingly, a recent retrospective study performed in Adelaide Dental Hospital analysing the oral lesions of HIV patients on HAART noted that combined anti-retroviral therapy may have increased LGE of patients compared to mono-HAART therapy (Freeman et al 2012 ). Oral candidiasis was the second most common oral lesion amongst the cohort.…”
