Organ donation following brain stem death after ventricular assist device implantation

Abstract: The availability of donor organs is the biggest limitation for lung transplantation, and a significant proportion of patients die on the waiting list. We describe a case of a 44-year-old lady who developed subarachnoid hemorrhage and cerebral edema on second postoperative day after left ventricular assist device implantation. She was declared brain stem dead 2 days later, and her organs were transplanted to suitable recipients on the waiting list for lung, liver and kidney transplantation.

“…Neurologic events during LVAD support are associated with significant morbidity and mortality rates and can cause brainstem death. A patient with diagnosed brain death who is supported by a VAD and has preserved end-organ function can and should be considered for donating organs [ 79 ], including the liver, kidneys, and lungs. In some cases, organ donation has also occurred after cardiac death.…”

“…However, intracranial hemorrhages may also occur, and some patients become brain dead due to hematoma expansion. Although there is no pulsatile flow or systolic blood pressure level to follow to conduct the BD exam in these patients, there have been reports of patients declared BD, 38,39 but details of the BD exam were not published.…”
