Organizing Pneumonia With Intense 68Ga-FAPI Uptake Mimicking Lung Cancer on 68Ga-FAPI PET/CT

Abstract: We report a case of organizing pneumonia, which revealed intense uptake on both 18 F-FDG and 68 Ga-FAPI (fibroblast activation protein inhibitor) PET/CT. Our findings indicate that organizing pneumonia should be taken into consideration when diagnosing a cancer-like pulmonary mass with intense 68 Ga-FAPI uptake.

“…In our study, mediastinal lymph nodes of two patients that were false positive on 18 F-FDG PET/CT due to anthracosis were negative on 68 Ga-FAPI PET/CT. Similar recent case reports support our findings [19,20]. Lymph node parameters less than 10 mm, low metabolic activity and small tumor volume can cause false negativity on 18 F-FDG PET/CT [18,21].…”

“…Imaging of isolated liver abscess lesions in 18 F-FDG PET/CT has been reported in the past 6–8 . Increased 68 Ga-FAPI activity has been previously reported in different infectious diseases including syphilis, 9 organizing pneumonia, 10 tuberculosis, 11 and cryptococcus 12 . Our case illustrates that 68 Ga-DOTA-FAPI PET/CT can be a 1-stop search for systemic occult lesions in patients with K. pneumoniae invasion syndrome.…”

“…68 Ga-DOTA-FAPI-04 is a novel PET agent that has been shown to be promising for imaging tumors 5,6 . In addition, 68 Ga-DOTA-FAPI-04 in multiple inflammatory lesions including tuberculosis, 7 arthritis, 8 chronic colitis, 9 esophagitis, 10 thyroiditis, 11 organizing pneumonia, 12 immunoglobulin G4–related disease, 13 SAPHO syndrome, 14 and dermatomyositis 15 has also been reported. In our case, 68 Ga-DOTA-FAPI-04 uptake was significantly increased in radioactive iodine–induced chronic parotitis.…”
