Introduction: Class III malocclusion is considered the most challenging discrepancies in orthodontic diagnosis and treatment planning. It is often difficult to classify borderline cases as surgical or non-surgical. The following case report is of a borderline Class III case with several missing maxillary premolars treated via an interdisciplinary approach. Aim: This clinical case highlights the importance of meticulous diagnosis to obtain optimal results in borderline Class III cases. The significance of an interdisciplinary approach in complex adult orthodontic cases was also discussed. Case Report: Given the complexity of the case, the treatment required a comprehensive interdisciplinary approach with the intervention of multiple specialties including periodontics, prosthodontics, orthodontics, oral surgery and maxillofacial surgery. The presurgical orthodontic stage was achieved in preparation for LeFort I maxillary advancement. Third molars extractions along with implant placement were implemented. Finally, crown placement and connective tissue graft were completed to achieve an optimal result. Total treatment time was 1.7 years (20 months). Patient’s profile and facial appearance were dramatically enhanced, and a stable functional Class II occlusion was attained despite the preexisting skeletal Class III. Conclusion: Borderline adult Class III cases require a delicate diagnostic approach to be able to distinguish a surgical from a non-surgical approach. Complex adult orthodontic cases require a diplomatic interdisciplinary approach from all required specialties in order to attain the most favorable results.

Introduction: This case report describes compensatory orthodontic treatment in a young patient aged 13 years. She presented with a class III skeletal malocclusion associated with mandibular laterognathy. The patient's main reason for consultation was the anterior cross bite and the aesthetics of her smile.Materials and Methods: The chosen treatment was therefore an orthodontic camouflage with the extraction of the first mandibular premolars and the second maxillary premolars, in order to catch a correct anterior articular and restore a good occlusal relationship, however, the mandibular laterognathy was camouflaged by means of dental compensations and also by correcting the deviation of the incisors medians through a class III mechanics with good anchorage management.Results: After 24 months of treatment, an ideal overjet and overbite associated with a Class I canine and molar relationship, was obtained, associated with a perfect coincidence of the interincisor medians.Conclusion: Class III skeletal cases can often be treated either by orthodontic camouflage or surgery. In our case study, the treatment adopted was orthodontic camouflage with extractions. The results of the treatment were satisfactory and the occlusal objectives were achieved. The final harmonious smile pleased the patient and improved her self-esteem and quality of life.

BackgroundOne of the most controversial issues in treatment planning of class III malocclusion patients is the choice between orthodontic camouflage and orthognathic surgery. Our aim was to delineate diagnostic measures in borderline class III cases for choosing proper treatment.MethodsThe pretreatment lateral cephalograms of 65 patients exhibiting moderate skeletal class III were analyzed. The camouflage group comprised of 36 patients with the mean age of 23.5 (SD 4.8), and the surgery group comprised of 29 patients with the mean age of 24.8 years (SD 3.1). The camouflage treatment consisted of flaring of the upper incisors and retraction of the lower incisors, and the surgical group was corrected by setback of the mandible, maxillary advancement, or bimaxillary surgery. Mann-Whitney U test was used to compare the variables between the two groups. Stepwise discriminant analysis was applied to identify the dentoskeletal variables that best separate the groups.ResultsHoldaway H angle and Wits appraisal were able to differentiate between the patients suitable for orthodontic camouflage or surgical treatment. Cases with a Holdaway angle greater than 10.3° and Wits appraisal greater than − 5.8 mm would be treated successfully by camouflage, while those with a Holdaway angle of less than 10.3° and with Wits appraisal less than − 5.8 mm can be treated surgically. Based on this model, 81.5% of our patients were properly classified.ConclusionsHoldaway H angle and Wits appraisal can be used as a critical diagnostic parameter for determining the treatment modality in class III borderline cases.
